About postmortem

postmortem
/em:postmortem — Honest analysis of what went wrong
Postmortem is an agent skill for an honest analysis of what went wrong after an event, such as a failed launch, a missed quarter, a feature that flopped, a bad hire or a deal that fell through. You run it with the command and a description of the event. Its aim is understanding rather than blame. It argues that most post-mortems fail in one of two ways: they turn into a blame session where people get scapegoated and the real causes go unexamined, or into a whitewash of vague action items that change nothing. The skill runs a blameless five-whys retrospective instead. It asks what actually happened, why, and which conditions made the outcome predictable, then shows what should change as a result. The output is meant to be a rigorous look at a system failure, so the same problem is less likely to return in a different form. It is part of the claude-skills collection by alirezarezvani and is listed on skills.sh as free.
